Register as a patient?
Please complete a registration form available at the surgery or download one here. Please print and complete the form and hand this to a member of the admin team.
Get to the surgery?
Please find directions to the surgery here.
Make an appointment / Speak to a Doctor or Nurse? You can eithering us, book an appointment online or visit the surgery where a member of the admin team can book you an appointment. Please ring us to book a telephone triage appointment. If your query is related to an administration matter please leave a message with the admin team. Please note all administration matters and paperwork are dealt with after surgery consulting times.
Order a repeat prescription?
You can either ring us, order it online or post your repeat prescription to the surgery. You must register first for the online repeat prescription ordering. Please speak to a member of the admin staff for more details. Please give at least 3 days notice for your monthly prescriptions.
Make a complaint?
We strive to provide you with a high standard of care. If you are not satisfied with any of our services please speak to our practice manager who will be pleased to help. If you want to complain in writing please collect a complaints leaflet from reception.
Get a sick note?
Sick notes or Fit notes are issued after absence from work for one working week where a self certification form which you can complete and hand to your employer. You can download a self certification form here.
